- Original and 2 copies of educational certificates/degrees (verified by District Education Officer for 8th class passed)
- Original and 2 copies of Computerized National Identity Card (CNIC) issued by NADRA
- Original and 2 copies of Domicile/Permanent Residential Certificate (PRC)
- 4 attested passport-sized photographs
- No Objection Certificate (NOC) from the head of department (for serving government/semi-government employees, duly signed)
